We are currently seeking a dedicated and passionate Qualified Social Worker to join our Wrexham - ATM - Looked After Children team. In this role, you will be responsible for providing high-quality support and intervention services to children and young people who are in the care system.
Key responsibilities and duties include:
Conducting assessments and developing care plans for looked after children. Working collaboratively with other professionals and agencies to ensure the best outcomes for children and young people. Advocating for the needs and rights of looked after children within the care system. Providing emotional support and guidance to children and young people in challenging situations. We are currently seeking an experienced and dedicated Qualified Social Worker (QSW) to join the Mental Health and Hospital Discharge Team in Wrexham. This is a fantastic locum opportunity offering a competitive hourly rate and the chance to make a significant impact within adult mental health services.
Key Responsibilities:
Undertake assessments and care planning under the Social Services and Well-being (Wales) Act (SSWBA).
Support hospital discharges in line with best practice and statutory guidance.
Complete Mental Health Act (MHA) and Mental Capacity Act (MCA) assessments where appropriate.
Work collaboratively with NHS staff, community teams, and service users to promote safe discharge and continuity of care.
Maintain accurate and timely records in line with statutory and service requirements.
Requirements:
Qualified Social Worker with current registration with Social Care Wales.
Proven experience in adult mental health and hospital discharge work.
Strong knowledge of:
Social Services and Well-being (Wales) Act (SSWBA)
Mental Health Act (MHA)
