We are seeking a dedicated and compassionate Social Worker to join our Adults Community Team in Manchester. As a Social Worker, you will play a crucial role in assessing needs, developing care plans, and providing ongoing support to individuals and their families.
Your key responsibilities will include:
Conducting comprehensive assessments of clients’ needs and risks. Developing and implementing tailored care plans in collaboration with clients and their families. Providing ongoing support and monitoring to ensure the effectiveness of care plans. Collaborating with other professionals and agencies to coordinate services and interventions. Maintaining accurate and up-to-date case records and documentation. Participating in team meetings and contributing to the development of best practices.To be considered for this role, you must possess the following essential qualifications and skills:
A recognized Social Work qualification (degree or equivalent). Registration with Social Work England. Experience working with adults in a community setting. Strong assessment and analytical skills. Excellent communication and interpersonal skills.
