The Mid & South Notts - Urgent & Emergency Care Team is seeking an experienced QSW to join our dynamic team. In this role, you will work collaboratively with various stakeholders to ensure the highest standards of care and support for individuals and families in crisis.
Your key responsibilities will include:
Assessing the needs of individuals and families in urgent situations and developing tailored support plans. Providing crisis intervention and support to vulnerable populations, including children and adults. Collaborating with healthcare professionals, law enforcement, and community organizations to coordinate services. Documenting case notes and maintaining accurate records in compliance with legal and organizational standards. Participating in team meetings and contributing to the development of best practices within the team.To be successful in this role, you must possess the following essential qualifications and skills:
A recognized social work qualification (e.g., BA/MA in Social Work). Current registration with Social Work England. Proven experience in social work, particularly in urgent and emergency care settings. Strong assessment and analytical skills, with the ability to make critical decisions under pressure. Join the dynamic Urgent and Emergency Care Team in Mid and South Nottinghamshire as we support adults through safe, timely hospital discharges and post-discharge recovery. You will play a vital role in the Discharge to Assess (D2A) pathway, helping individuals transition from hospital care to short-term support services aimed at maximizing independence and well-being.
About the Role:
Based within integrated Transfer of Care Hubs located in hospital settings, you will work closely with health professionals, patients, and carers to assess needs, coordinate support, and ensure continuity of care.
Key Responsibilities:
Undertake strength-based Care & Support Assessments
Complete Mental Capacity and Best Interests Assessments
Safeguard vulnerable adults through robust practice and intervention
Triage within the Transfer of Care Hub
Plan and review reablement packages
Provide tailored carer support
Participate in Decision Support Tool (DST) meetings
You'll Work Closely With:
Multi-disciplinary health and care professionals
Community reablement teams
Hospital discharge coordinators
Families and carers
Requirements:
Qualified Social Worker (SWE registered)
Experience in hospital discharge or urgent/emergency care settings
Strong knowledge of the Care Act, Mental Capacity Act, and safeguarding procedures
Ability to work in a fast-paced, collaborative environment
