We are currently seeking an experienced Qualified Social Worker (QSW) to join the Adult Social Care Team in Middlesbrough. This is an excellent opportunity to contribute to a forward-thinking local authority focused on promoting independence, well-being, and person-centred care.

About the Role:
You will work within a multi-disciplinary team to assess needs, develop support plans, and safeguard adults at risk. This role involves working with a wide range of adults with complex needs, including physical disabilities, learning disabilities, mental health issues, and older adults.

Key Responsibilities:

Conduct Care Act assessments and reviews

Undertake Mental Capacity and Best Interests Assessments

Support safeguarding enquiries under Section 42

Work collaboratively with families, carers, and partner agencies

Develop strength-based care and support plans

Ensure compliance with relevant legislation and policy

Requirements:

Qualified Social Worker with active Social Work England registration

Previous experience in adult social care

Strong working knowledge of the Care Act, Mental Capacity Act, and safeguarding procedures

Excellent communication and report writing skills

Ability to manage a varied caseload effectively

You will be working within a supportive team environment focused on promoting independence, safeguarding wellbeing, and delivering strength-based assessments in line with the Care Act 2014.

Key Responsibilities:
Undertake assessments and develop care and support plans tailored to individual needs

Work with adults, their families, and multi-agency partners to ensure positive outcomes

Carry out safeguarding enquiries and manage risks appropriately

Support hospital discharges and prevent unnecessary admissions

Maintain accurate, up-to-date case records and reports

Requirements:
Social Work qualification (BA/MA/DipSW or equivalent)

Registration with Social Work England

Minimum of 2 years' experience in adult services

Strong knowledge of relevant legislation including the Care Act and Mental Capacity Act

Ability to work flexibly, independently, and as part of a team
