An exciting opportunity has arisen for an experienced Qualified Social Worker to join the Learning Disability Team in Newcastle. This is a fantastic opportunity to work with adults with learning disabilities and autistic adults, supporting them to achieve positive outcomes through person-centred assessments and reviews.
The RoleYou will be responsible for undertaking statutory assessments and reviews, working closely with individuals, their families, carers, and partner agencies to ensure that support plans meet assessed needs and promote independence, choice, and wellbeing.
Key ResponsibilitiesComplete strengths-based assessments for adults with learning disabilities and autistic adults.Undertake scheduled reviews of care and support plans.Develop person-centred support plans to achieve positive outcomes.Work collaboratively with health professionals, families, carers, and partner agencies.Manage and prioritise a varied caseload effectively.Ensure all assessments and case records are completed accurately and in line with statutory requirements.Promote independence while safeguarding vulnerable adults and ensuring best practice is followed.Essential RequirementsSocial Work qualification (Degree, DipSW or equivalent).Registration with Social Work England.Experience working with Autistic Adults and/or Adults with Learning Disabilities is highly desirable.Experience completing statutory assessments and reviews within Adult Social Care.Good knowledge of the Care Act 2014, Mental Capacity Act, and Safeguarding Adults legislation.Excellent communication, assessment, and case management skills.Ability to work independently and as part of a multidisciplinary team.Benefits of Working with Eden Brown SynergyDBS check processed and paid for by Eden Brown Synergy.£300 referral bonus when you refer a colleague or friend.Bonus for finding your own job.Twice-weekly payroll.Dedicated recruitment consultant to support you throughout your job search, placement and beyond.If you are interested in this position, please send your CV to Shristi Yadav at S.yadav@ edenbrownsynergy.com or call 0114 700 3826.
