Nottinghamshire is seeking 2 experienced Qualified Social Workers to join our newly established Mental Health Hospital Discharge (HD) Team. This is an exciting opportunity to play a key role in shaping a fast-paced, multi-disciplinary service supporting timely and safe discharges from acute mental health settings.

Pay rate: dependent on experince

Key Responsibilities:

Undertake mental health and social care assessments to support discharge planning.

Work closely with multi-disciplinary teams (MDT), hospital wards, and community services.

Ensure discharges are person-centred, legally compliant, and timely.

Apply Care Act 2014, Mental Health Act 1983 (amended 2007), and related legislation effectively.

Essential Requirements:

Social Work England registration

Minimum 2 years' post-qualified experience in mental health and hospital discharge settings.

Proven ability to work within acute inpatient services and multi-agency teams.

Strong assessment, planning, and discharge coordination skills.
