Role Overview:
We are seeking a skilled and motivated Band 6 Physiotherapist to join the Discharge to Assess (D2A) service based within the Community Services team at Musgrove Park Hospital. This role supports patients who have been recently discharged from hospital, enabling safe recovery and reablement within their own homes.

The post holder will work collaboratively with D2A-trained provider agencies to assess patients, deliver targeted rehabilitation, and set realistic SMART goals to maximise independence and support long-term care planning.

Key Responsibilities:
Deliver community-based physiotherapy to adults post-discharge

Carry out functional assessments within the patients home environment

Set patient-led SMART goals and monitor progress toward independence

Work closely with reablement care providers to facilitate seamless support

Identify and recommend ongoing support needs or referrals where appropriate

Familiarity with local community care structures and health/social care integration

Additional Notes:
The role is part of a dynamic service designed to support short-term reablement following hospital admission. Flexibility is essential, as while the service is organised by geographical clusters, cross-county travel may be required to meet patient needs. A full-time candidate is preferred, though part-time availability can be discussed.
