A well-respected Local Authority in the Caerphilly area is seeking an experienced Qualified Social Worker to join their Community Team. This is an excellent opportunity for a skilled practitioner looking to take on a varied caseload within a supportive and established service.
You will play a key role in completing assessments and managing ongoing care arrangements to support adults within the community to live safely and independently.
Key Responsibilities:Undertake strengths-based social care assessmentsManage a diverse community-based caseloadArrange and review Packages of CareCoordinate Direct PaymentsSupport placements in Extra Care Housing, Supported Living, and Residential/Nursing homesWork collaboratively with multi-disciplinary professionals and partner agenciesBenefits:Hybrid working model (3 days per week in the office)Competitive pay ratesLong-term contract (minimum 6 months)Free on-site parkingOpportunity to work within a supportive and well-regarded Local Authority teamAbout You:You will be a qualified and registered Social Worker with experience in adult services and community-based practice.
