As a Qualified Social Worker in our Adult Care Team, your key responsibilities will include:

Conducting comprehensive assessments of individuals’ needs and developing tailored care plans. Providing ongoing support and guidance to service users, ensuring their rights and preferences are respected. Collaborating with healthcare providers, community organizations, and other stakeholders to coordinate services and resources. Monitoring and reviewing care plans to ensure the effectiveness of interventions and making adjustments as necessary. Maintaining accurate and up-to-date case records in compliance with local policies and regulations.

To be successful in this role, you must possess the following essential qualifications and skills:

A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Proven experience in adult social care, with a strong understanding of relevant legislation and best practices. Excellent communication and interpersonal skills, with the ability to build rapport with diverse individuals. Strong analytical and problem-solving abilities, with a commitment to continuous professional development.
