We are currently seeking a dedicated and experienced Adult's Social Worker to join our Locality Team in Liverpool. As a key member of the team, you will be responsible for providing high-quality social work services to adults in need of support and intervention.

Key responsibilities and duties:

Conducting assessments and developing care plans for adults with complex needs Working collaboratively with multi-disciplinary teams to ensure holistic support for service users Providing advocacy and support to empower individuals to make informed decisions about their care Monitoring and reviewing the effectiveness of interventions and adjusting plans as necessary Participating in safeguarding procedures and taking appropriate action to protect vulnerable adults

Essential qualifications and skills:

Qualified Social Worker with current registration with the HCPC Demonstrated experience in adult social work, preferably within a locality team setting Strong assessment and analytical skills, with the ability to formulate and implement care plans Excellent communication and interpersonal skills, with the ability to build rapport with service users and stakeholders Sound knowledge of relevant legislation and policies governing adult social care
