We are seeking a passionate and dedicated Qualified Social Worker (QSW) to join our Learning Disabilities team in Somerset. This role offers an exciting opportunity to make a meaningful impact on the lives of individuals with learning disabilities and their families. As a key member of our multidisciplinary team, you will provide high-quality social work support, ensuring that individuals receive the care and resources they need to thrive in their communities.
Your key responsibilities will include:
Conducting comprehensive assessments of individuals with learning disabilities to identify their needs and strengths. Developing and implementing personalized support plans in collaboration with clients, families, and other professionals. Providing ongoing support and advocacy to individuals and their families, helping them navigate services and resources. Maintaining accurate and up-to-date case records while adhering to safeguarding policies and procedures. Participating in multidisciplinary meetings and contributing to service development initiatives.To be successful in this role, you must possess the following qualifications and skills:
A recognized social work qualification (BA/MA in Social Work or equivalent). Current registration with Social Work England. Experience working with individuals with learning disabilities and a solid understanding of relevant legislation. Excellent communication and interpersonal skills, with the ability to build rapport with clients and stakeholders.
