We are currently seeking a dedicated and experienced Adult's Social Worker to join our Locality Team in Liverpool. As a key member of our team, you will be responsible for providing high-quality social work services to adults in need of support and intervention.

Key responsibilities and duties:

Conducting assessments and developing care plans for adults with complex needs Working collaboratively with multi-disciplinary teams to ensure holistic support for service users Providing advocacy and support to empower individuals to make informed decisions about their care Participating in safeguarding procedures and risk assessments to ensure the safety and well-being of vulnerable adults Engaging with community resources and services to enhance the support available to service users

Essential qualifications and skills:

Qualified Social Worker with current registration with Social Work England Demonstrated experience in adult social work, ideally within a locality team setting Strong assessment and care planning skills, with the ability to work autonomously and as part of a team Excellent communication and interpersonal skills, with a compassionate and empathetic approach Sound knowledge of relevant legislation and policies related to adult social care
