We are currently seeking a dedicated and experienced Qualified Social Worker to join our Mental Health Social Work Discharge Team in West Sussex on a part-time basis. As a key member of the team, you will play a crucial role in supporting service users in their transition from hospital to community care.
Key responsibilities and duties:
Conducting assessments and developing care plans for service users being discharged from mental health facilities. Collaborating with healthcare professionals, families, and community resources to ensure a smooth and effective discharge process. Providing ongoing support and intervention to service users to promote their mental health and well-being in the community. Maintaining accurate and up-to-date records in accordance with regulatory standards and best practices.Essential qualifications and skills:
Qualified Social Worker with current registration with the Health and Care Professions Council (HCPC). Experience working in mental health settings and a strong understanding of relevant legislation and policies. Excellent communication and interpersonal skills, with the ability to build rapport with diverse individuals and groups.
