We are currently seeking a dedicated and experienced Qualified Social Worker to join our Hospital Team in North Lincoln. As a key member of the team, you will be responsible for providing high-quality social work services to patients and their families within a hospital setting.

Key responsibilities and duties include:

Conducting assessments of patients' social care needs and developing care plans Providing support and guidance to patients and their families to facilitate their understanding of the care process Liaising with healthcare professionals, external agencies, and community resources to coordinate holistic care for patients Participating in multi-disciplinary meetings and case conferences to ensure effective communication and collaboration Advocating for the rights and best interests of patients to promote their well-being and safeguarding

The ideal candidate will possess the following essential qualifications and skills:

Qualified Social Worker with current registration with Social Work England Demonstrated experience in a hospital or healthcare setting, preferably within a similar role Strong assessment, communication, and interpersonal skills Ability to work effectively under pressure and manage a complex caseload Knowledge of relevant legislation, policies, and procedures related to social work practice

In addition to the above requirements, candidates with experience in mental health, safeguarding, or discharge planning will be highly regarded. We are currently seeking a dedicated and experienced Senior Qualified Social Worker (QSW) to join our team in North Lincoln. In this role, you will be responsible for providing high-quality social work services within localities, making a positive impact on the lives of vulnerable individuals and families.

Key responsibilities and duties:

Undertaking complex assessments and creating care plans to support service users. Providing professional guidance and support to junior staff members. Collaborating with multi-disciplinary teams and external agencies to ensure holistic care for clients. Participating in case conferences, reviews, and court proceedings as required. Ensuring all work is carried out in line with relevant legislation and best practice standards.

Essential qualifications and skills:

Qualified Social Worker with relevant registration and at least 2 years of post-qualifying experience. Demonstrated knowledge of safeguarding procedures and experience working with vulnerable populations. Strong communication and interpersonal skills, with the ability to build rapport with clients and stakeholders. Excellent organizational abilities and the capacity to manage a demanding workload effectively.
