We are currently seeking a dedicated and passionate Qualified Social Worker (QSW) to join our team in Wolverhampton, specializing in working with individuals with Learning Disabilities (LD) and Physical Disabilities (PD). As a key member of our team, you will play a crucial role in providing support and guidance to vulnerable individuals in need of social care services.
Key responsibilities and duties:
Assessing the needs of individuals with LD and PD to develop personalized care plans Providing advocacy and support to empower clients to make informed decisions Collaborating with multi-disciplinary teams to ensure holistic care and support Conducting risk assessments and safeguarding vulnerable adults Participating in case conferences and reviews to monitor progress and adjust interventions as neededEssential qualifications and skills:
Qualified Social Worker with relevant experience in working with individuals with LD and PD Registered with Social Work England and up-to-date with CPD requirements Strong communication and interpersonal skills to engage effectively with clients and stakeholders Ability to work autonomously and as part of a team in a fast-paced environment Knowledge of relevant legislation and policies governing social care servicesAdditional requirements and benefits:
