We are currently seeking a dedicated and experienced Qualified Social Worker to join our Transitions Team in Wolverhampton. As a key member of the team, you will play a crucial role in supporting young adults as they transition from child to adult services, ensuring they receive the necessary care and support to thrive in their new phase of life.

Key responsibilities and duties:

Assessing the needs of young adults approaching transition age and developing individualized transition plans Working collaboratively with service users, families, and partner agencies to coordinate support services Providing advocacy and guidance to empower young adults in making informed decisions about their future Conducting regular reviews and evaluations to monitor progress and adjust support plans as needed Ensuring compliance with relevant legislation and safeguarding procedures

Essential qualifications and skills:

Qualified Social Worker with a valid HCPC registration Experience working with young adults in a social care setting, particularly in transitions services Strong assessment and case management skills Excellent communication and interpersonal abilities Ability to work effectively within a multidisciplinary team
