We are seeking a Part-Time Adult Social Worker to join the Intermediate Tier Service in Warrington. This integrated health and social care team delivers short-term support, focused on helping individuals regain independence and return to their previous level of functioning after crisis or hospital stays.

Key Responsibilities:

Complete Care Act assessments and outcome-focused support planning

Work with health professionals to deliver joined-up care

Support timely discharges and avoid unnecessary hospital admissions

Participate in care reviews and provide holistic support to clients

Promote recovery and reduce long-term care dependency

Requirements:

Qualified Social Worker with current SWE registration

Adult social care and intermediate care experience essential

Enhanced DBS, 3 years of references, employment gaps checked

Relevant training and qualifications as per JD/PS

We are currently seeking an experienced Adult Social Worker to join Warrington's Intermediate Tier Service on a full-time, hybrid basis. This is a multi-disciplinary, integrated health and social care service supporting people to live as independently as possible, particularly during times of crisis, hospital discharge, or intermediate need.

Key Responsibilities:

Undertake strengths-based assessments under the Care Act 2014

Promote recovery and independence through short-term support

Collaborate with health colleagues, therapists, and reablement staff

Prevent unnecessary hospital admissions and reduce long-term care needs

Deliver person-centred, timely interventions in a fast-paced team

Requirements:

Qualified Social Worker with current Social Work England registration

Proven experience in intermediate care or reablement services

Ability to work effectively in a multidisciplinary team

Enhanced DBS, 3 years of references, employment gaps checked

Relevant qualifications as per JD/PS
