We are currently seeking an experienced Social Worker to join our Adult Front Door Team in Worcestershire. As a key member of the team, you will be responsible for providing high-quality social work services to adults in need of support and intervention.

Key responsibilities and duties:

Assessing the needs of vulnerable adults and developing care plans Working collaboratively with other professionals and agencies to ensure holistic support Providing advocacy and support to clients to empower them in decision-making processes Conducting risk assessments and safeguarding vulnerable adults Participating in multidisciplinary meetings and case conferences

Essential qualifications and skills:

Qualified Social Worker with HCPC registration Proven experience working in adult social care, preferably within a front door team Strong assessment and analytical skills Excellent communication and interpersonal skills Ability to work under pressure and manage a complex caseload
