Worcestershire County Council is looking for an experienced Occupational Therapist to join their Hospital Discharge Team.
Key Responsibilities:
Deliver a specialist OT service to promote independence and reduce the need for care.
Use equipment, assistive technology, and manual handling techniques.
Review post-hospital discharge care packages to ensure effective and timely support.
This role is ideal for someone confident in hospital discharge work and passionate about making a difference in adult social care.
