We are seeking experienced and qualified Social Workers to join our dynamic team to carry out Care Act compliant assessments and support complex Mental Capacity and Best Interest decision-making within acute hospital settings. These roles will play a crucial part in ensuring timely, effective, and person-centred care planning.
Key Responsibilities:
Conduct Care Act compliant assessments.
Handle complex Mental Capacity and Best Interest decisions.
Provide expert support for hospital discharges and contribute to multi-disciplinary planning.
Maintain high-quality case recording and prepare comprehensive financial panel papers.
Prioritize casework effectively in fast-paced hospital environments.
Collaborate with multi-agency teams to deliver integrated care solutions.
Utilize Hull Royal Infirmary as a working base with flexibility for home working.
Locations:
Scarborough Hospital: 1 Social Worker
Hull Royal Infirmary: 2 Social Workers
Supporting discharges from Scunthorpe, Doncaster, and Goole: 2 Social Workers
Requirements:
Qualified Social Worker with relevant registration (e.g., Social Work England).
Proven experience with hospital discharge processes and Care Act assessments.
Strong understanding of the Continuing Healthcare (CHC) criteria and DST checklists.
Excellent time management skills with the ability to manage a continuous workflow.
High standard of recording and clear, concise decision-making abilities.
Access to reliable transport for travel between hospital sites.
Experience in multi-agency working is desirable.
