Northumberland County Council is seeking a dedicated and experienced Qualified Social Worker to join our Community Team. This role offers an excellent opportunity to support adults in the community by promoting independence and delivering high-quality care services.
Key Responsibilities:
Conduct person-centered assessments of adults and their carers to determine care and support needs.
Develop and implement care plans to promote service user independence and well-being.
Provide timely interventions and support to prevent escalation of needs.
Work collaboratively with service users, families, healthcare providers, and other professionals to ensure coordinated care.
Assess and manage risks while ensuring safeguarding responsibilities are met.
Monitor and review care plans to ensure they remain effective and responsive to changing needs.
Maintain accurate and timely records in compliance with statutory requirements.
Requirements:
Degree in Social Work and registration with Social Work England.
Proven experience working within community-based adult social care services.
Strong knowledge of relevant legislation, including the Care Act 2014 and Mental Capacity Act 2005.
Ability to conduct comprehensive assessments and manage complex cases.
Excellent communication, negotiation, and interpersonal skills.
Commitment to a strengths-based approach in practice.
Full UK driving license and access to a vehicle (preferred).
