Are you an experienced leader looking to take on a weekend-focused role that ensures smooth and effective hospital discharges? Southwark Council is seeking a Deputy Team Manager to oversee the Transfers of Care process during critical weekend periods.
About the Role:
? Operational Leadership: Manage day-to-day operations, triage referrals, allocate cases, and authorize assessments to keep care transitions running smoothly.
? Expert Guidance: Provide leadership on complex discharges, safeguarding cases, and mental capacity assessments, supporting your team in making sound decisions.
? Collaborative Networking: Act as the lead manager for hospital discharges at Kings and St Thomas Hospital, working closely with healthcare professionals to ensure timely and safe discharges.
? Team Supervision: Oversee 4-5 qualified social workers, providing reflective supervision, mentorship, and professional development support.
What We're Looking For:
? Deep Knowledge: Strong understanding of the Care Act, Mental Capacity Act, and Continuing Healthcare processes, with proven experience in applying these frameworks.
? Leadership & Development: Experience leading multi-disciplinary teams, safeguarding meetings, and reflective supervision. A background in ASYE or Practice Education is a plus!
? Problem-Solving & Decision-Making: Ability to lead with confidence, think critically under pressure, and ensure the best outcomes for residents.
