This is a frontline role, ideal for a dynamic practitioner who can deliver timely, strengths-based responses to adults requiring support, safeguarding, or guidance at the first point of contact.
Key Responsibilities:
Act as the initial point of contact for new referrals into Adult Social Care
Conduct strengths-based conversations and initial assessments to determine eligibility and next steps
Triage and manage a high volume of cases, ensuring swift and appropriate responses
Promote independence and early intervention to reduce longer-term dependency on services
Collaborate effectively with multi-agency partners to deliver coordinated support
Apply relevant legislation including the Care Act 2014, Mental Capacity Act, and safeguarding frameworks
Requirements:
Social Work England registration

Strong knowledge of relevant legislation and safeguarding procedures

Key Responsibilities

Undertake assessments of new referrals to the service.
Liaise with service users, carers, and families via phone and in-person.
Take part in duty cover on a rota basis.
Care coordinate cases in line with CPA.
Participate in regular multidisciplinary team (MDT) meetings.
Essential Requirements
Qualified Social Worker with current registration.
Experience working in community mental health settings, ideally with older adults.

An exciting opportunity has arisen for a dedicated social worker to deliver an effective social work service to children with disabilities and their families, informed by current research, legislation and departmental policy. Candidates must have experience of working within a Disabled Childrens Team with knowledge of Child in Need, Children Looked After and Safeguarding Plans. The suitable candidate must have court experience, and the ability to work within a fast pace work environment.
The teams undertake all of the work that traditionally involves a mainstream team. This involves assessing the need and identifying services that will enhance the life of the disabled child and their family. A large proportion of the work within CWD services is to identify need and provide services that remove barriers so that the child can reach his/her full potential. By working in partnership with the child's family, we help enable the parents/carers to continue to meet the needs of their child while alleviating some of the stress that this will involve. Social workers attend various panels in order to advocate for our young people who require a Residential placement or who require a direct payment to help support their care.
