Derbyshire County Council is seeking an experienced Qualified Social Worker (QSW) to join the Chesterfield Royal Hospital Team.
About the Role
The Chesterfield Royal Hospital Team primarily provides short-term assessment and support to maximise independence and prevent unnecessary hospital admissions. Our goal is to enable individuals to continue living in their own homes for as long as possible.
As a member of our team, you will be expected to work within tight discharge timescales, making this an exciting opportunity for professionals who thrive in a fast-paced, integrated multi-disciplinary setting.
Key Responsibilities
Conduct assessments and provide short-term support to individuals and families.
Work collaboratively with health professionals, agencies, and community services.
Ensure that discharge plans are implemented effectively and efficiently.
Promote independence and well-being, reducing the need for long-term care interventions.
Adhere to relevant legislation, including the Care Act 2014, Mental Capacity Act, and Making Safeguarding Personal principles.
Maintain accurate and up-to-date case records.
What We're Looking For
A Qualified Social Worker (QSW) with HCPC/Social Work England registration.
Strong knowledge of the Care Act 2014, Mental Capacity Act, and safeguarding principles.
Experience working in a hospital discharge setting or multi-disciplinary team (desirable but not essential).
Ability to work under pressure, managing competing priorities effectively.
Commitment to a person-centred and strengths-based approach.
Excellent communication and relationship-building skills.
