Lincolnshire County Council is seeking an experienced and committed Qualified Social Worker to join our Adult Frailty & Long-Term Conditions (AFLTC) team. This role is essential in delivering statutory adult social care services to adults with a wide range of care and support needs, focusing on strengths-based and person-centred practice.
As a Social Worker in this team, you will manage a complex and varied caseload, contribute to safeguarding adults at risk, and work collaboratively with partners across health and social care to achieve the best possible outcomes for individuals and communities.
Key Responsibilities:
Assess, plan, implement, and review care and support plans for adults with complex needs
Conduct Care Act 2014-compliant needs assessments using strengths-based approaches
Work with safeguarding leads on enquiries and place individuals at the heart of safeguarding practice
Develop creative and cost-effective care packages that meet eligible needs
Offer expert advice and mentoring to Adult Care practitioners
Act as Practice Educator for social work students or mentor ASYE social workers
Deputise for Lead Practitioner and represent the team at multi-disciplinary meetings
Supervise up to four non-registered practitioners, addressing work allocation, support, conduct, and development
Identify and address wider non-social care needs (e.g., housing, health), signposting appropriately
Maintain accurate records, contribute to reports, and provide court evidence when required
Adhere to safeguarding procedures and council policies
Operate within delegated budgets and contribute to service planning and improvement
What We're Looking For:
Qualified Social Worker, registered with Social Work England
Extensive experience working with adults with frailty and physical disabilities
Skilled in Mental Capacity Assessments, CHC processes, and Direct Payments
Confident using Mosaic case management system
Strong knowledge of the Care Act 2014, safeguarding processes, and strengths-based practice
Excellent organisational, mentoring, and communication skills
Willingness to travel across Lincolnshire and occasionally outside the county
