Location: Leeds
Rate: Up to £35 per hour
Hours: Part-time - 18.5 hours per week (6 hours x 2 days, 6.5 hours x 1 day)
Contract: 12-Month Maternity Cover
An exciting opportunity has arisen for a Qualified Social Worker to join the Continuing Health Care team in Leeds on a part-time basis.
Role Overview:
You will be responsible for:
Completing CHC assessments, including Checklists and Decision Support Tools (DSTs)
Working collaboratively with NHS partners and adult social care teams
Ensuring compliance with CHC framework and legislation
Requirements:
Qualified Social Worker with Social Work England registration
Proven experience in Continuing Health Care assessments
Ability to work in the office one day per week (Wednesdays)
Full UK driving licence and access to a vehicle
