Join our dedicated Care Connect Team in North Lincolnshire as an Adult's Social Worker! We are seeking a passionate and qualified individual to support adults in navigating their care needs and enhancing their quality of life. In this pivotal role, you will work collaboratively with individuals, families, and multidisciplinary teams to assess needs, develop care plans, and provide ongoing support to those in our community.
Your key responsibilities will include:
Conducting comprehensive assessments of individuals' social care needs and risks. Developing, implementing, and reviewing personalized care plans in partnership with service users. Providing information, advice, and support to individuals and their families regarding available services. Working closely with health professionals, community organizations, and other stakeholders to ensure a holistic approach to care. Maintaining accurate and timely records in accordance with legal and organizational standards.The ideal candidate will possess the following qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Demonstrable experience in adult social work, particularly in community settings. Strong communication and interpersonal skills, with the ability to build rapport with service users and their families.
