Your key responsibilities will include:

  • Conducting comprehensive assessments of individuals' social care needs and risks.
  • Developing, implementing, and reviewing personalized care plans in partnership with service users.
  • Providing information, advice, and support to individuals and their families regarding available services.
  • Working closely with health professionals, community organizations, and other stakeholders to ensure a holistic approach to care.
  • Maintaining accurate and timely records in accordance with legal and organizational standards.

The ideal candidate will possess the following qualifications and skills:

  • A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
  • Registration with Social Work England.
  • Demonstrable experience in adult social work, particularly in community settings.
  • Strong communication and interpersonal skills, with the ability to build rapport with service users and their families.
  • Excellent organizational skills and the ability to manage a varied caseload effectively.
