Are you a dedicated and compassionate social worker looking to make a meaningful impact in the lives of adults? We are seeking a qualified Adult's Social Worker to join our dynamic Hospital Team in Oxfordshire. In this role, you will work collaboratively with healthcare professionals to assess, plan, and implement care for adults with a range of complex needs, ensuring their safety and well-being during their hospital stay and post-discharge.
Your key responsibilities will include:
Conducting comprehensive assessments of patients’ social, emotional, and practical needs. Developing and implementing care plans in partnership with patients, families, and multidisciplinary teams. Providing support and guidance to patients and their families, ensuring they understand available resources and options. Advocating for patients’ rights and best interests within the hospital setting. Participating in case reviews and contributing to discharge planning to ensure continuity of care.The ideal candidate will possess the following essential qualifications and skills:
A recognized social work qualification (e.g., CQSW, DipSW, or equivalent). Registered with Social Work England. Experience in adult social work, particularly within a hospital or health care setting. Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals.
