Your key responsibilities will include:
- Conducting comprehensive assessments of patients’ social, emotional, and practical needs.
- Developing and implementing care plans in partnership with patients, families, and multidisciplinary teams.
- Providing support and guidance to patients and their families, ensuring they understand available resources and options.
- Advocating for patients’ rights and best interests within the hospital setting.
- Participating in case reviews and contributing to discharge planning to ensure continuity of care.
The ideal candidate will possess the following essential qualifications and skills:
- A recognized social work qualification (e.g., CQSW, DipSW, or equivalent).
- Registered with Social Work England.
- Experience in adult social work, particularly within a hospital or health care setting.
- Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals.
- Excellent organizational skills and the ability to manage a caseload effectively.
