We are currently seeking a dedicated and experienced Adult's Social Worker to join our Locality Team at the Royal Borough of Kensington and Chelsea. As a key member of the team, you will be responsible for providing high-quality social work services to vulnerable adults within the community.

Key responsibilities and duties include:

Conducting assessments and developing care plans for adults in need of support Working collaboratively with other professionals, agencies, and service users to ensure holistic care Participating in multi-disciplinary meetings and reviews to monitor progress and make adjustments as needed Providing advocacy and support to empower individuals to make informed decisions about their care Ensuring compliance with relevant legislation, policies, and procedures to safeguard the well-being of service users

Essential qualifications and skills:

Qualified Social Worker with a valid HCPC registration Demonstrated experience in adult social work, preferably within a locality team setting Strong assessment and case management skills Excellent communication and interpersonal abilities to engage effectively with diverse individuals and groups Ability to work autonomously and as part of a team in a fast-paced environment
