We are currently seeking a dedicated and experienced Adult's Social Worker to join our Locality Team at the Royal Borough of Kensington and Chelsea. As a key member of the team, you will be responsible for providing high-quality social work services to vulnerable adults within the community.

Key responsibilities and duties include:

  • Conducting assessments and developing care plans for adults in need of support
  • Working collaboratively with other professionals, agencies, and service users to ensure holistic care
  • Participating in multi-disciplinary meetings and reviews to monitor progress and make adjustments as needed
  • Providing advocacy and support to empower individuals to make informed decisions about their care
  • Ensuring compliance with relevant legislation, policies, and procedures to safeguard the well-being of service users

Essential qualifications and skills:

  • Qualified Social Worker with a valid HCPC registration
  • Demonstrated experience in adult social work, preferably within a locality team setting
  • Strong assessment and case management skills
  • Excellent communication and interpersonal abilities to engage effectively with diverse individuals and groups
  • Ability to work autonomously and as part of a team in a fast-paced environment
