We are currently seeking a dedicated and passionate Adult's Social Worker to join our Reablement Team in the vibrant London Borough of Islington. As a key member of our team, you will play a crucial role in supporting adults in the community to regain their independence and improve their quality of life.

Key responsibilities and duties:

Undertake assessments of individuals' needs and develop personalized care plans Provide practical support and guidance to service users to help them achieve their reablement goals Liaise with healthcare professionals, families, and other stakeholders to ensure coordinated care delivery Monitor progress and evaluate the effectiveness of interventions, making adjustments as necessary Maintain accurate and up-to-date records in accordance with statutory requirements

Essential qualifications and skills:

Qualified Social Worker with current registration with Social Work England Demonstrated experience working with adults in a social care setting Strong assessment and care planning skills Excellent communication and interpersonal skills Ability to work effectively as part of a multidisciplinary team

Additional requirements:

Experience in reablement or rehabilitation services is highly desirable Knowledge of relevant legislation and policies governing adult social care Commitment to continuous professional development and learning Competitive salary and benefits package Opportunities for career progression and further training
