Adult's SW - Reablement Team - London Borough of Islington
We are currently seeking a dedicated and passionate Adult's Social Worker to join our Reablement Team in the vibrant London Borough of Islington. As a key member of our team, you will play a crucial role in supporting adults in the community to regain their independence and improve their quality of life.
Key responsibilities and duties:
- Undertake assessments of individuals' needs and develop personalized care plans
- Provide practical support and guidance to service users to help them achieve their reablement goals
- Liaise with healthcare professionals, families, and other stakeholders to ensure coordinated care delivery
- Monitor progress and evaluate the effectiveness of interventions, making adjustments as necessary
- Maintain accurate and up-to-date records in accordance with statutory requirements
Essential qualifications and skills:
- Qualified Social Worker with current registration with Social Work England
- Demonstrated experience working with adults in a social care setting
- Strong assessment and care planning skills
- Excellent communication and interpersonal skills
- Ability to work effectively as part of a multidisciplinary team
Additional requirements:
- Experience in reablement or rehabilitation services is highly desirable
- Knowledge of relevant legislation and policies governing adult social care
