We are currently seeking a passionate and dedicated Adult's TM - LD/MH Team Social Worker to join our team in Hammersmith & Fulham. In this role, you will be responsible for providing high-quality social work services to adults with learning disabilities and mental health needs.

Key responsibilities and duties include:

Conducting assessments and developing care plans for individuals based on their specific needs and goals Collaborating with multi-disciplinary teams to coordinate and deliver support services Advocating for clients to ensure their rights and best interests are upheld Providing crisis intervention and support during challenging situations Participating in case conferences and reviews to monitor progress and adjust interventions as needed

The ideal candidate will possess the following qualifications and skills:

Qualified Social Worker with current registration with the Health and Care Professions Council (HCPC) Demonstrated experience working with adults with learning disabilities and mental health issues Strong assessment and care planning skills Excellent communication and interpersonal abilities Ability to work effectively in a fast-paced and demanding environment
