Adult's TM - LD/MH Team - Hammersmith & Fulham

We are currently seeking a passionate and dedicated Adult's TM - LD/MH Team Social Worker to join our team in Hammersmith & Fulham. In this role, you will be responsible for providing high-quality social work services to adults with learning disabilities and mental health needs.

Key responsibilities and duties include:

  • Conducting assessments and developing care plans for individuals based on their specific needs and goals
  • Collaborating with multi-disciplinary teams to coordinate and deliver support services
  • Advocating for clients to ensure their rights and best interests are upheld
  • Providing crisis intervention and support during challenging situations
  • Participating in case conferences and reviews to monitor progress and adjust interventions as needed

The ideal candidate will possess the following qualifications and skills:

  • Qualified Social Worker with current registration with the Health and Care Professions Council (HCPC)
  • Demonstrated experience working with adults with learning disabilities and mental health issues
  • Strong assessment and care planning skills
  • Excellent communication and interpersonal abilities
  • Ability to work effectively in a fast-paced and demanding environment

Additional requirements for this role include a valid driver's license and access to a vehicle for home visits.
