Are you a passionate and dedicated social worker looking to make a real difference in the lives of adults in Herefordshire? We are seeking an experienced Adult's Team Manager to join our Locality Team. In this pivotal role, you will lead a team of qualified social workers and support staff, ensuring high-quality assessments and interventions are delivered to adults in need of care and support.

Your key responsibilities will include:

Managing and supervising a team of social workers, providing guidance and support to ensure effective case management. Overseeing the assessment and planning processes for adults with complex needs, ensuring compliance with statutory requirements. Developing and implementing service improvements and best practices to enhance the quality of care provided. Building strong relationships with partner agencies and stakeholders to promote collaborative working. Ensuring the team meets performance targets and contributes to the overall objectives of the Locality Team.

To be successful in this role, you will need to possess the following essential qualifications and skills:

A relevant social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Current registration with Social Work England. Significant post-qualification experience in adult social work, including managerial experience. Strong leadership and communication skills, with the ability to motivate and inspire a team. In-depth knowledge of relevant legislation and best practice in adult social care.

We are currently seeking a dedicated and experienced Adult's TM - Locality Team Social Worker to join our team in Herefordshire. In this role, you will be responsible for providing high-quality social work services to adults in need of support and intervention within the locality team.

Key responsibilities and duties include:

Undertaking assessments of individuals' needs and risks Developing care and support plans in collaboration with service users and their families Providing ongoing support and monitoring to ensure the effectiveness of interventions Liaising with other professionals and agencies to coordinate holistic care for service users Participating in multi-disciplinary meetings and case conferences

The ideal candidate will possess the following essential qualifications and skills:

Qualified Social Worker with HCPC registration Demonstrated experience in adult social work, preferably within a locality team setting Strong assessment and analytical skills Excellent communication and interpersonal abilities Ability to work effectively under pressure and manage a complex caseload

Additional requirements for this role include a valid driver's license and access to a vehicle for travel within the locality.
