Adult's TM - Locality Team - Herefordshire
We are currently seeking a dedicated and experienced Adult's TM - Locality Team Social Worker to join our team in Herefordshire. In this role, you will be responsible for providing high-quality social work services to adults in need of support and intervention within the locality team.
Key responsibilities and duties include:
- Undertaking assessments of individuals' needs and risks
- Developing care and support plans in collaboration with service users and their families
- Providing ongoing support and monitoring to ensure the effectiveness of interventions
- Liaising with other professionals and agencies to coordinate holistic care for service users
- Participating in multi-disciplinary meetings and case conferences
The ideal candidate will possess the following essential qualifications and skills:
- Qualified Social Worker with HCPC registration
- Demonstrated experience in adult social work, preferably within a locality team setting
- Strong assessment and analytical skills
- Excellent communication and interpersonal abilities
- Ability to work effectively under pressure and manage a complex caseload
