Your key responsibilities will include:
Conducting assessments to identify the needs of children and young people in transition. Developing and implementing tailored support plans in collaboration with families and stakeholders. Providing ongoing support and guidance to children, young people, and their families throughout the transition process. Monitoring and reviewing progress, ensuring that interventions are effective and responsive to changing needs. Maintaining accurate and timely records in compliance with local policies and safeguarding regulations.The ideal candidate will possess the following essential qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registered with Social Work England. Experience in working with children and families, particularly in a transitions or leaving care context. Strong communication and interpersonal skills, with the ability to build rapport with children and families.
