Join our dedicated Transitions Team in Worcestershire as a Children's Social Worker, where you will play a pivotal role in supporting young people as they transition into adulthood. Your expertise will be vital in ensuring that children and young adults receive the care and guidance they need during this crucial phase of their lives. You will work collaboratively with families, educational institutions, and other professionals to promote positive outcomes and empower children to thrive.

Your key responsibilities will include:

Conducting assessments to identify the needs of children and young people in transition. Developing and implementing tailored support plans in collaboration with families and stakeholders. Providing ongoing support and guidance to children, young people, and their families throughout the transition process. Monitoring and reviewing progress, ensuring that interventions are effective and responsive to changing needs. Maintaining accurate and timely records in compliance with local policies and safeguarding regulations.

The ideal candidate will possess the following essential qualifications and skills:

A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registered with Social Work England. Experience in working with children and families, particularly in a transitions or leaving care context. Strong communication and interpersonal skills, with the ability to build rapport with children and families. Excellent organizational skills and the ability to manage a diverse caseload effectively.

Join our dedicated Transitions Team in Worcestershire as a Children’s Social Worker! We are seeking a passionate and skilled individual to support young people in their transition to adulthood. In this role, you will work collaboratively with children, families, and various stakeholders to ensure the best outcomes for our young clients. Your expertise will contribute to creating meaningful plans that empower young individuals to thrive as they move into adulthood.

Your key responsibilities will include:

Conducting comprehensive assessments to identify the needs and strengths of children and young people. Developing and implementing care plans that promote independence, resilience, and well-being. Building strong relationships with children, families, and professionals to facilitate effective support networks. Monitoring and reviewing progress, ensuring that services are responsive and tailored to individual needs. Advocating for the rights and voices of young people within the local community and beyond.

To be successful in this role, you must possess the following essential qualifications and skills:

A recognized social work qualification (e.g., BA/MA in Social Work, CQSW, DipSW). Current registration with Social Work England. Experience working with children and young people, particularly in transition services. Strong communication and interpersonal skills, with the ability to engage and build rapport with diverse individuals. Sound knowledge of relevant legislation and safeguarding practices.
