- Providing ongoing support and guidance to children, young people, and their families throughout the transition process.
- Monitoring and reviewing progress, ensuring that interventions are effective and responsive to changing needs.
- Maintaining accurate and timely records in compliance with local policies and safeguarding regulations.
The ideal candidate will possess the following essential qualifications and skills:
- A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
- Registered with Social Work England.
- Experience in working with children and families, particularly in a transitions or leaving care context.
- Strong communication and interpersonal skills, with the ability to build rapport with children and families.
- Excellent organizational skills and the ability to manage a diverse caseload effectively.
