Herefordshire Council currently looking for an experienced and dedicated Social Worker to join their Children With Disabilities team

Location: Plough Lane, Hereford

Pay Rate: £30.57 per hour (Umbrella)

Key Responsibilities:

Undertake holistic assessments for children and young people with disabilities and additional needs.Develop, implement, and review tailored support plans to meet individual needs and promote positive outcomes.Work directly with children, young people, and their families to provide guidance, support, and advocacy.Coordinate services and interventions, ensuring the right support is provided at the right time.Build strong, trusting relationships with families, promoting partnership working and empowerment.Collaborate effectively with multi-agency partners, including education, health, and community services.Promote independence, wellbeing, and opportunities for children and young people to participate in their communities.Ensure the child's voice, lived experience, and best interests remain central to all planning and decision-making.Safeguard children and young people, ensuring their safety and welfare at all times.Contribute to service development and improvement, supporting positive change for children with disabilities across the local authority.

Essential Requirements:

Social Work Qualification - Degree or equivalentAt least 3 years' experience as a social worker within children's servicesSocial Work England RegistrationDriving license and access to own vehicle
