Herefordshire - QSW - Children with Disability Team
Herefordshire Council currently looking for an experienced and dedicated Social Worker to join their Children With Disabilities team
Location: Plough Lane, Hereford
Pay Rate: £30.57 per hour (Umbrella)
Key Responsibilities:
- Undertake holistic assessments for children and young people with disabilities and additional needs.
- Develop, implement, and review tailored support plans to meet individual needs and promote positive outcomes.
- Work directly with children, young people, and their families to provide guidance, support, and advocacy.
- Coordinate services and interventions, ensuring the right support is provided at the right time.
- Build strong, trusting relationships with families, promoting partnership working and empowerment.
- Collaborate effectively with multi-agency partners, including education, health, and community services.
- Promote independence, wellbeing, and opportunities for children and young people to participate in their communities.
- Ensure the child's voice, lived experience, and best interests remain central to all planning and decision-making.
- Safeguard children and young people, ensuring their safety and welfare at all times.
- Contribute to service development and improvement, supporting positive change for children with disabilities across the local authority.
Essential Requirements:
- Social Work Qualification - Degree or equivalent
- At least 3 years' experience as a social worker within children's services
- Social Work England Registration
- Driving license and access to own vehicle
