Herefordshire Council currently looking for an experienced and dedicated Social Worker to join their Children With Disabilities team

Location: Plough Lane, Hereford

Pay Rate: £30.57 per hour (Umbrella)

Key Responsibilities:

  • Undertake holistic assessments for children and young people with disabilities and additional needs.
  • Develop, implement, and review tailored support plans to meet individual needs and promote positive outcomes.
  • Work directly with children, young people, and their families to provide guidance, support, and advocacy.
  • Coordinate services and interventions, ensuring the right support is provided at the right time.
  • Build strong, trusting relationships with families, promoting partnership working and empowerment.
  • Collaborate effectively with multi-agency partners, including education, health, and community services.
  • Promote independence, wellbeing, and opportunities for children and young people to participate in their communities.
  • Ensure the child's voice, lived experience, and best interests remain central to all planning and decision-making.
  • Safeguard children and young people, ensuring their safety and welfare at all times.
  • Contribute to service development and improvement, supporting positive change for children with disabilities across the local authority.

Essential Requirements:

  • Social Work Qualification - Degree or equivalent
  • At least 3 years' experience as a social worker within children's services
  • Social Work England Registration
  • Driving license and access to own vehicle
