We are currently seeking a dedicated and experienced Qualified Social Worker (QSW) to join our team in Conwy, specializing in Older People & Hospital services. As a key member of our social work team, you will play a crucial role in supporting and advocating for the well-being of older individuals in hospital settings.

Key responsibilities and duties include:

Assessing the needs of older people in hospital and developing tailored care plans Providing emotional support and guidance to patients and their families Collaborating with healthcare professionals to ensure holistic care for older individuals Advocating for the rights and best interests of older people within the hospital setting Participating in multi-disciplinary team meetings and case conferences

The ideal candidate for this position will possess:

A recognized qualification in Social Work and registration with Social Work England Demonstrated experience working with older people and a sound understanding of issues affecting this demographic Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals Excellent organizational skills and the ability to manage a complex caseload effectively Commitment to upholding ethical standards and promoting social justice

Pay Rate: £35.98 per hour
Hours: 37 hours per week (Monday to Friday)
Contract Length: Approx. 12 weeks (with potential extension)
Regulatory Body: Social Care Wales

Overview:
Conwy County Borough Council is currently seeking a Qualified Social Worker for a short-term assignment within our Older People & Hospital Social Work Services. This is an exciting opportunity to join a proactive, person-centred team supporting older adults at a critical point in their lives.

Key Responsibilities:

Deliver a high-quality social work service to older people who are vulnerable or in need of support

Work collaboratively with citizens, carers, and families to promote independence and resilience

Ensure the individual and their support network are central to care planning and decision-making

Liaise with multi-disciplinary teams within hospital and community settings to coordinate effective discharge planning and support packages

Requirements:

Social Work qualification with a minimum of 3 years post-qualification experience

Registered with Social Care Wales

Experience working in Older People and Hospital Social Work settings

Strong interpersonal and team-working skills

Competence using relevant technology and social work systems

Ability to meet essential criteria outlined in the full Job Description and Person Specification (available on request)

Additional Information:

This is an office-based role located at Eirias Park, Colwyn Bay

Candidates must live locally or within a commutable distance to Conwy County

The post is initially for 12 weeks but may be extended depending on service needs
