We are currently seeking a passionate and dedicated Qualified Social Worker to join our team at the Cheshire East - TM - Children with Disabilities Hub. As a key member of our team, you will play a crucial role in providing support and services to children with disabilities and their families.
Key responsibilities and duties:
Assessing the needs of children with disabilities and developing care plans Working collaboratively with families, healthcare professionals, and other agencies to ensure the best outcomes for the children Providing advocacy and support to children and families to navigate the social care system Participating in multidisciplinary team meetings and case conferences Monitoring and reviewing the progress of children in care and making necessary adjustments to support plansEssential qualifications and skills:
Qualified Social Worker with a valid registration with the Health and Care Professions Council (HCPC) Demonstrated experience working with children with disabilities and their families Strong assessment and case management skills Excellent communication and interpersonal skills Ability to work effectively in a fast-paced and challenging environmentAdditional requirements and benefits:
