We are currently seeking a dedicated and experienced Adult's Social Worker to join our Mental Health Team in North Lincolnshire. As a key member of the team, you will be responsible for providing high-quality social work services to adults with mental health needs in the community.

Key responsibilities and duties include:

Conducting assessments and developing care plans for individuals with complex mental health needs. Working collaboratively with multi-disciplinary teams to ensure holistic support for service users. Providing advocacy and support to empower individuals in making informed decisions about their care and treatment. Participating in safeguarding procedures and taking appropriate actions to protect vulnerable adults. Engaging with service users, their families, and carers to promote independence and well-being.

The ideal candidate will possess the following essential qualifications and skills:

Qualified Social Worker with current registration with the HCPC. Demonstrated experience working with adults with mental health needs in a social work setting. Strong assessment and care planning skills, with the ability to work autonomously and as part of a team. Excellent communication and interpersonal skills, with a compassionate and non-judgmental approach. Knowledge of relevant legislation and policies related to mental health and social care.

Additional requirements for this role include a valid driver's license and access to a vehicle for work-related travel within the community.
