Adult's SW - MH Team - North Lincolnshire
We are currently seeking a dedicated and experienced Adult's Social Worker to join our Mental Health Team in North Lincolnshire. As a key member of the team, you will be responsible for providing high-quality social work services to adults with mental health needs in the community.
Key responsibilities and duties include:
- Conducting assessments and developing care plans for individuals with complex mental health needs.
- Working collaboratively with multi-disciplinary teams to ensure holistic support for service users.
- Providing advocacy and support to empower individuals in making informed decisions about their care and treatment.
- Participating in safeguarding procedures and taking appropriate actions to protect vulnerable adults.
- Engaging with service users, their families, and carers to promote independence and well-being.
The ideal candidate will possess the following essential qualifications and skills:
- Qualified Social Worker with current registration with the HCPC.
- Demonstrated experience working with adults with mental health needs in a social work setting.
- Strong assessment and care planning skills, with the ability to work autonomously and as part of a team.
- Excellent communication and interpersonal skills, with a compassionate and non-judgmental approach.
- Knowledge of relevant legislation and policies related to mental health and social care.
