Newcastle City Council is seeking an experienced Qualified Social Worker (QSW) to join a dedicated project team within Adult Social Care. This 6-month role will provide vital capacity to review Continuing Health Care (CHC) cases and support effective decision-making between the local authority and Integrated Care Board (ICB).
Role Purpose:
The postholder will contribute to the review of CHC cases by preparing for and attending DST (Decision Support Tool) meetings, managing disputes, and completing Care Act Assessments where required. The role is crucial in supporting joint working and ensuring a smooth transition of responsibilities and funding arrangements when CHC eligibility is not met.
Key Responsibilities:
Arrange and confirm attendance at DST/MDT meetings.
Prepare for DST meetings by gathering information from Eclipse, care providers, and service users/families.
Attend and contribute to DST/MDT meetings.
Record outcomes on Eclipse where PHN (Primary Health Need) is agreed - no further action required.
Where PHN is not agreed and there is disagreement:
Clearly set out points of disagreement in writing to the ICB.
Attend reconvened DSTs if necessary.
Draft dispute letters for escalation to the Service Manager.
Where PHN is not agreed and no dispute:
Complete Care Act Assessment and Support Plan.
Submit financial assessment referral.
Transfer responsibility and funding to Newcastle City Council.
Provide follow-up contact with the individual/family to explain the outcome and next steps.
Essential Requirements:
Social Work qualification and current registration with Social Work England (SWE)
Demonstrable experience working with CHC processes, DST meetings, and Care Act Assessments
Understanding of the interface between local authority and NHS funding responsibilities
Strong case management and communication skills
Ability to work independently and manage competing priorities in a time-limited project environment
