We are looking for a passionate, motivated and experienced Social Worker to join our Mental Health Team. This is a small, well-established, dedicated social care service for people with severe and enduring mental health conditions.
The mental health social work teams provide specialist services for adults over 18. The teams support people to recover from periods of difficulty with their mental health and for people to stay as independent as possible by providing support that is person-centred which gives people choice and control in their lives.
As a Mental Health Social Worker, you will also be involved in assessing, coordinating, and arranging care for service users with mental health problems, including dementia, both in the community and in the hospital.
You will need to be Registered with Social Work England / your appropriate Professional body. Experience of working in a health or social care setting is essential, along with excellent communication skills. You will need to demonstrate passion and enthusiasm for integrated working and an understanding of how we can best engage with our local communities and other partners in a meaningful way.
Working model: Hybrid (50% of working hours from home)
Hours per week: 37
Location: County Hall, Trowbridge

Wiltshire Council is looking to appoint an experienced and dynamic Qualified Social Worker to join a collaborative initiative with SFT NHS Foundation Trust. This project aims to enhance hospital discharge processes through early assessment and discharge planning, ultimately reducing delays and improving outcomes for patients.
About the Role:
As part of a dedicated team, you will be responsible for conducting early assessments of patients aged 65 and over who have been admitted within the last 48 hours across three wards. The focus is to establish clear discharge proposals, identify any complexities, and initiate early planning-either within the Trust or the community-prior to patients becoming classified as having No Criteria to Reside (NC2R).
You will contribute to setting realistic expectations for patients, healthcare staff, and partner organisations regarding discharge pathways and anticipated resource requirements. This proactive approach is intended to reduce the length of hospital stays for patients who are medically fit for discharge.
Key Responsibilities:
Undertake timely and thorough assessments of newly admitted patients (within 48 hours)
Collaborate closely with multidisciplinary teams across health and social care
Develop discharge plans based on individual needs, aiming to reduce delays post-NC2R
Collect and report data to support ongoing evaluation and potential service expansion
Contribute to a sustainable and measurable model that informs future discharge services

An exciting opportunity has arisen for an experienced social worker to join the Integrated Front Door Team in Trowbridge. The team are responsible for providing a consistent and timely response to new safeguarding concerns out of hours, 7 days a week, 365 days a year and is essential to service delivery. The successful candidate will be a part of a seven-week rolling rota across day times, evenings and weekends providing the opportunity for them to gain the experience in both our Multi Agency Safeguarding Team and Emergency Duty Service. This is a rare role which allows for direct contact with children, without being case holding. Much of your time will be spent on the telephone, with emergency visits as needed.
The successful candidate will thrive on creative thinking and problem solving and enjoy working autonomously. You will possess excellent communication skills, have the ability to identify risk and prioritise accordingly whilst remaining calm and know when to escalate concerns to your team leader.
