An excellent opportunity has become available for an experienced Qualified Social Worker to join the Hospital Discharge Team in Newcastle. This hybrid role offers a rewarding opportunity to support adults in the community following hospital discharge, helping them achieve positive outcomes and maintain their independence.
The RoleWorking with adults 4-10 weeks post-discharge, you will carry out strengths-based assessments, develop support plans, and ensure individuals receive the appropriate care and services to meet their ongoing needs. The role involves a combination of office-based, home-based, and community visits.
Key ResponsibilitiesComplete post-discharge assessments for adults within the community.Develop and review person-centred support plans.Undertake Mental Capacity Act (MCA) assessments and Best Interest decision-making where appropriate.Manage safeguarding concerns in line with statutory guidance and local procedures.Participate in Continuing Healthcare (CHC) assessments and processes.Work collaboratively with health professionals, community services, and partner agencies to ensure safe and effective outcomes.Maintain accurate and timely case records in accordance with professional standards.Effectively manage and prioritise a varied caseload.Essential RequirementsSocial Work qualification (Degree, DipSW or equivalent).Registration with Social Work England.Experience working within Adult Social Care, preferably Hospital Discharge or Community Teams.Sound knowledge of the Care Act 2014, Mental Capacity Act, Safeguarding Adults, and Continuing Healthcare (CHC) processes.Excellent assessment, communication, and case management skills.Ability to work independently as well as part of a multidisciplinary team.Benefits of Working with Eden Brown SynergyDBS check processed and paid for by Eden Brown Synergy.£300 referral bonus when you refer a colleague or friend.Bonus for finding your own job.Twice-weekly payroll.Dedicated recruitment consultant to support you throughout your job search, placement and beyond.If you are interested in this position, please send your CV to Shristi Yadav at S.yadav@ edenbrownsynergy.com or call 0114 700 3826.

Maintaining accurate and timely documentation of assessments, plans, and follow-up actions in compliance with legal and organizational standards.To be considered for this role, you must possess a valid Social Work qualification and be registered with Social Work England.
