Qualified Social Worker - Hospital Discharge Team (Hybrid)
Location: Newcastle
Rate: £27.56 per hour
Working Pattern: Hybrid (Office, Home & Community-Based)
An excellent opportunity has become available for an experienced Qualified Social Worker to join the Hospital Discharge Team in Newcastle. This hybrid role offers a rewarding opportunity to support adults in the community following hospital discharge, helping them achieve positive outcomes and maintain their independence.
The Role
Working with adults 4-10 weeks post-discharge, you will carry out strengths-based assessments, develop support plans, and ensure individuals receive the appropriate care and services to meet their ongoing needs. The role involves a combination of office-based, home-based, and community visits.
Key Responsibilities
- Complete post-discharge assessments for adults within the community.
- Develop and review person-centred support plans.
- Undertake Mental Capacity Act (MCA) assessments and Best Interest decision-making where appropriate.
- Manage safeguarding concerns in line with statutory guidance and local procedures.
- Participate in Continuing Healthcare (CHC) assessments and processes.
- Work collaboratively with health professionals, community services, and partner agencies to ensure safe and effective outcomes.
- Maintain accurate and timely case records in accordance with professional standards.
- Effectively manage and prioritise a varied caseload.
Essential Requirements
- Social Work qualification (Degree, DipSW or equivalent).
- Registration with Social Work England.
- Experience working within Adult Social Care, preferably Hospital Discharge or Community Teams.
- Sound knowledge of the Care Act 2014, Mental Capacity Act, Safeguarding Adults, and Continuing Healthcare (CHC) processes.
- Excellent assessment, communication, and case management skills.
- Ability to work independently as well as part of a multidisciplinary team.
